Permadeath or Death means death is a roleplay concept, where you don't load saves after your in-game character dies. In other words, if you die - it's over, you start a new character.
There can be different additional House Rules, like High Difficulty Level, Survival Mods and No Fast Travel. This is all up to you, but the bottom line is you don't load saves after you die, no matter how painful it is. There are people, who make significant progress in the game, and then die due to some nonsense. They don't reload. They are real heroes.
